DietitianIowa.com is operated by El Leon LLC. Fresh Fit Meals is the service provider that receives consultation requests and provides the consultations, meal programs, and professional nutrition services described on this website.
This policy describes information handled through DietitianIowa.com. It does not replace the separate privacy practices of Fresh Fit Meals or any external service you choose to visit.
2. Information You Provide
The public consultation form asks for limited contact and scheduling information:
- first and last name;
- email address and phone number;
- preferred contact method;
- preferred Fresh Fit Meals location;
- a general goal category and, when Other is selected, an optional short explanation;
- an optional best time to contact you; and
- a required consent confirmation allowing Fresh Fit Meals to contact you about the request.
The optional Plan Finder request uses the same basic contact information. It may also collect one or more general goal categories, an optional short goal detail, broad meal preferences, food-sensitivity preferences, the desired level of prepared-meal support, pickup or delivery preference, a pickup location or delivery address and ZIP, and optional Additional Notes. These answers help Fresh Fit Meals follow up; they do not create a meal order or confirm delivery eligibility.
Inline delivery checks may collect a ZIP code to determine whether it is currently listed in the Fresh Fit Meals delivery area. These checks do not create a lead by themselves.
The Plan Finder is not a medical intake form.
The consent confirmation is checked when the form is submitted; the current lead table does not store it as a separate lead field. Authorized staff may later add operational follow-up notes. The admin interface tells staff not to enter medical history or other sensitive clinical information in those notes.
Do not submit diagnoses, medical history, medication lists, insurance information or other insurance details, Social Security numbers, payment-card information or other payment-card data, or internal medical records.
3. Information Collected Automatically
The application uses PHP session information to support form security, duplicate-submission protection, rate limiting, temporary form state, and signed-in admin sessions.
On a visitor's first eligible page view, the application may record first-touch campaign values supplied in the URL: UTM source, medium, campaign, term, and content. It also records the landing-page path and a valid HTTP or HTTPS referring-page URL when one is supplied by the browser.
The hosting environment and application logs may also contain ordinary technical request or error information, such as timestamps, requested paths, Internet Protocol addresses in standard server logs, and generic delivery or error events. The application does not build browser fingerprints, advertising profiles, or cross-site tracking records.
4. How Information Is Used
Information may be used to:
- respond to consultation and Plan Finder requests;
- route requests to Fresh Fit Meals;
- send an internal lead notification;
- operate, protect, and troubleshoot the website;
- understand which campaigns, referring pages, or landing pages generated a request;
- manage follow-up status and limited operational notes; and
- maintain records reasonably needed for site and request operations.
5. How Information Is Shared
Lead-request information may be available to Fresh Fit Meals, authorized El Leon LLC personnel who operate the website, and hosting, email, or technical vendors when reasonably necessary to operate the service. Information may also be disclosed when required by law or a valid legal process.
The application may send lead details to a configured notification email address. When email delivery is disabled, unavailable, or unsuccessful, the same notification details may be written to a private server-side log for operational follow-up.
DietitianIowa.com does not sell or rent consultation-request information.
7. Data Retention
No fixed automatic deletion period is currently configured in the application. Consultation records, operational notes, notification logs, and relevant technical records are intended to be kept only as long as reasonably needed for the purposes described here, operational needs, dispute handling, and legal obligations.
Deletion or correction requests may be subject to operational, backup, security, or legal requirements. A specific retention schedule and deletion workflow have not yet been configured.
8. Security
The application uses reasonable safeguards appropriate to its current scope, including prepared database statements, form-security tokens, submission rate limiting, restricted admin access, secure session-cookie settings in production, output escaping, and browser security headers.
No website or transmission method can promise absolute security. Visitors should use the public form only for the limited contact and scheduling information it requests.
9. Your Choices
- You may browse without submitting the consultation form or Plan Finder.
- You can block or remove cookies through your browser settings, although doing so may affect form or session functions.
- You may ask about a submitted request or request correction or deletion, subject to operational and legal requirements.
- You can choose not to follow an external booking or provider link.
